Resident Voter Verification Form
Election Day: Tuesday, February 17, 2026 — 9:00 a.m. to Noon

Purpose of this Form
This form confirms eligibility to vote in the February 17, 2026 University Park Recreation District (UPRD) election, as required by the District Charter.
Completion is required for:
- New Owners not yet verified through property appraiser records, and
- Residents who are not listed on the deed but meet UPRD residency requirements.
Completion is also recommended for all homeowners who wish to have their 2026 election ballot sent to an address other than their University Park residence so that District Management can record a secondary mailing address for ballot delivery.
Eligibility Requirements
You are eligible to vote if you:
- Are 18 years or older, and
- Either:
- Own a property in University Park (as shown on the recorded deed), or
- Reside at a University Park address for at least 183 days in 2025.
Owner Eligibility
Owners listed on the deed are automatically eligible to vote. Verification is based on Manatee County Property Appraiser records.
If your ownership is not yet recorded, you may confirm eligibility by providing:
- A recorded deed or deed of trust listing all owners/trustees/principals
Resident Eligibility (Not Listed on the Deed)
Residents (including renters, long-term guests, family members, or trustees/beneficiaries living in a trust-owned property) may vote if they meet residency requirements.
To qualify, you must:
- Provide your full name and University Park address.
- Complete the sworn residency statement within this form (affirming 183+ days of residence in 2025).
- Upload proof of residency, such as:
- Driver’s license showing your University Park address,
- Property tax or utility bill in your name, or
- Government- or county-issued document confirming your University Park residence.
➤ All proof of residency documents must be dated at least 183 days before December 31, 2025 (on or before July 2, 2025).
If you cannot upload valid proof of residency documentation, you must submit a notarized Residency Affidavit.

Deadline: Resident voter verification must be completed by December 31, 2025.
Ownership Classifications
- Multiple Owners: Each owner of record receives a ballot.
- Trusts: One designated trustee receives the owner ballot.
- Corporations or Entities: One designated principal receives the owner ballot.
- Renters/Guests: May vote as residents if they meet residency requirements.

Information submitted through this verification process is used solely for determining voter eligibility and administering the district election. All documentation will be handled in accordance with applicable Florida public records laws.
